Sacramental Records Held at St. Hugo of the Hills

To obtain copies of your Baptismal, First Eucharist, Confirmation or Marriage certificates, please complete the form linked below.   Certificates will be completed within 7 to 10 days and mailed to your residence or directly to the church requesting your certificate(s). 

Please note, if you are over the age of 18, you must request your own certificate and provide a copy of a valid, signed photo ID (i.e., driver’s license). Parents are allowed to request certificates for a minor up until the age of 18. Certificates cannot be faxed or emailed due to the raised seal necessary on each certificate.
